IV. Enterprise Name Change

       1. Applications for changing the original enterprise name to a new one

       2. Approval by the administrative department of the original enterprise

       3. Approval by provincial pesticide control (municipal) institutes

       4. Business license of the new enterprise (duplicate)

       5. Registration certificates of the original enterprise (original)
